No paper or pen required. Use of this electronic data entry form is optional and available to all California professional drivers. You may submit up to two (2) runs at a time. Route information is sent directly to your transportation department. And a copy will be sent to your email. Please note: If you close the screen before clicking the submit button, the data will be erased.
